Plan Details
- Plan cannot be used in conjunction with another dental discount plan or insurance policy
- Plan is not transferable to any other patient, family member, or friend
- No additional discounts can apply
- Plan cannot be used for services related to injuries covered under workers’ compensation
- Plan cannot be used for dental treatments in progress
- Plan is non-refundable
- Plan expires one year from the date of purchase, regardless of usage
